Renewal/Re-issue of Passport for Adults
Change of Name:
Alias Name
Only real/ official name of the applicant is printed on the Passport. Supporting ID proofs (Aadhaar, PAN, Voter ID etc.) for current name to be provided
Maiden to married name (change in surname)-
Marriage Certificate issued by competent authority
OR
Joint Declaration as prescribed in Annexure J (link)
Reissue of passport to married women with maiden name [Women who are married, not divorced or widowed, but wants to get passport in maiden name ]
Clippings of two newspapers [one newspaper in India and one in UAE] or the Gazette notification of the concerned State Government (Click here for advertisement format)
At least two public documents issued in the desired name. [In case of NRI applicants and their dependent family members who have been staying abroad on long term permanent or work visa/permit - two documents issued by the local Government are also acceptable]
[Note:- This clause would not be applicable for temporary residents and visitors from India.]
Change of Name/Addition of surname
Clippings of two newspapers [one prominent newspaper in India and one local newspaper in UAE] or the Gazette notification of the concerned State Government (Click here for advertisement format)
At least two public documents issued in the desired name. [In case of NRI applicants and their dependent family members who have been staying abroad on long term permanent or work visa/permit - two documents issued by the local Government are also acceptable]
[Note:- This clause would not be applicable for temporary residents and visitors from India.]
It may also be noted that addition of surname to the given name will also be treated as name change.
Shifting of words of name/Change of sequence of words of name/changing arrangements of various parts of existing name
(e.g., Parmeshwaran Shiva Kumaran to Shiva Parmeshwaran Kumaran)
At least two public documents issued in the desired name. [In case of NRI applicants and their dependent family members who have been staying abroad on long term permanent or work visa/permit - two documents issued by the local Government are also acceptable]
Minor corrections in name
(e.g., D’Souza to De Souza or Sanjeev to Sanjiv)

Splitting of existing name in given name and surname
(eg. If an applicant held a passport in the name of SANJU MOHAN PRASAD with surname field blank, he may be allowed to enter SANJU MOHAN or SANJU as the first name and PRASAD or MOHAN PRASAD as surname.)
At least two public documents issued in the desired name. [In case of NRI applicants and their dependent family members who have been staying abroad on long term permanent or work visa/permit - two documents issued by the local Government are also acceptable]
8. Initials in the name should not be written
The Passport application should be indicate the expanded name of the applicant as per the passport rules. Please refer to the instructions for filling the passport application form.
9. Change of Place or Date of Birth
Applicants need to take prior approval of the Consulate General of India, Dubai before submitting the application with BLS international. Refer Circular for guidelines in regard to change/correction of date of birth entries in the passport of an applicant already held by him/her.

10. Change of address (Passport Printable address)
Any of the following documents may be accepted as Proof of Residence in India-
Aadhaar Card/ e-Aadhar/ Letter containing the unique Aadhar number
Applicant’s current and valid ration card
Driving License
Election ID card
Electricity bill
Gas Connection Bill
Income Tax Assessment Order
Marriage Certificate issued by the Registrar of Marriages along with address proof of spouse in respect of newly married women
Parents’ address proof in case of minors (if address is other than printed on passport)
Passport of son/ daughter in case of dependent parents
Photo Passbook of running Bank Account (Scheduled Public Sector Banks, Scheduled Private Sector Indian Banks and Regional Rural Banks only)
Proof of Address (POA) issued by the India Post, Department of Posts
Rent Agreement
Spouse's passport copy/address proof if want to take address of spouse
Telephone (landline or post-paid mobile bill)
Water Bill
If applicant wants to get UAE Address on Passport then the following documents may be accepted-
DEWA/SEWA bill and title deed under applicant’s/spouse/ parent’s name only.
Address Proof in India for the purpose of verification
11.Correction of Father/Mother Name
[Minor change in father’s/mother’s name Additional documents required]
Father’s passport copy or Mother’s passport copy as the case may be or other ID proof with correct name.
OR
Applicant’s Birth Certificate
OR
Educational document of applicant such as School Leaving Certificate, Yearly Mark sheet, certifying Father’s/Mother’s name as the case may be.
[Major/complete change in father’s name]
Father’s/Mother’s passport copy as the case may be
OR
Applicant’s Birth Certificate
OR
Educational document of applicant such as School Leaving Certificate, Yearly Marksheet, certifying Father’s/Mother’s name as the case may be.
AND
One and Same Certificate/ Divergence Certificate issued by the State/Central Government in India OR a letter from the Tehsildar/ District Administration of the local district in India certifying that both the names (of father/mother as the case may be) belong to same person.
12. Change in the name of deceased parents
There is no law under which a person, not even the legal heirs of a deceased, can change the name of deceased persons. Admittedly, a deceased person has throughout his life used the name for all official transactions during his life period and now his legal heirs/descendants cannot, for their convenience whatsoever, be allowed to change the name of a deceased

13.Deletion of Spouse name
Divorce Decree issued/approved by Indian Court
[Note: - As per the extant guidelines issued by Government of India, foreign divorce judgment is not valid even if it’s duly apostilled/authenticated by foreign Government or Indian Embassy/Consulate abroad for purpose of grant of any passport service.
In case of mutually agreed or mutually properly contested divorce cases, an application must be submitted by the parties at the competent Indian civil court and a declaratory order authenticating and confirming that the foreign divorce decree is in accordance with Indian law must be obtained by the concerned parties, before submitting the mutual/contested foreign divorce decree as supporting documents for grant of any passport service.]
14.Addition of Spouse name
Marriage Certificate issued by competent authority

15.Lost Passport
Photocopy of the lost passport (of first, last, address page if separate page, any other endorsement pages and visa page with additional booklets, if any.
FIR/Police Certificate/Translated version in English

Cases if the valid UAE residence visa page is not available then Entry/Exit paper issued by UAE immigration authority with legal English translation is required.
Consent of both the parents of the applicant, in case of minors.
16.Damaged Passport
Damaged passport, in original, with a clear photocopy (of first, last, address page (if separate page), any other endorsement pages and visa page with additional booklets, if any.

18.Indian Passports issued for travel to Israel
For Business purpose –
Invitation letter from company in Israel.
Photo testified letter and Trade License from local Company in Dubai.
For Tourism purpose (sponsored by employer) –
Hotel Booking and Ticket Booking is required.
Photo testified letter from sponsor in UAE and passport copy of the sponsor.
Sworn Affidavit duly attested by the Consulate General of India, Dubai.
19.Removal of ECR
All persons having educational qualification of matriculation (pass) and above or persons processing two years diploma from any institute recognized by the National Council for Vocational Training (NCVT) or State Council of Vocational Training (SCVT), or persons holding three years diploma/equivalent degree from institutions, such as Polytechnics recognized by the Central/State Governments of India.
ECR passport holder completed 3 years abroad, other than India – Application accepted with copies of entry/exit stamp and previous visas.
All persons above the age of 50 years and children up to the age of 18 years.
20.Change of signature/ Appearance
No additional documents are required.
