Issuance of Fresh Passport

For New born child name initials are not allowed in the birth certificate. If there is any initial in the name, then the birth certificate has to be changed with full name. The Passport application should be indicate the expanded name of the applicant as per the passport rules. Please refer to the instructions for filling the passport application form.

Both the parents need to submit the form with following documents-

1. MHA birth registration form 

2. Birth certificate issued by the Ministry of Health, UAE along with authentic English translation of the Birth Certificate if the original birth certificate is in Arabic. Birth Certificate attested by MoFA, UAE is required if it is not having QR code for verification.

3. Copy of marriage certificate of parents issued by competent authority, if spouse name is not endorsed in Passport

4. Passports of the parents 

5. Visas of the parents 

6.  In case the newborn is more than one year, parents need to submit the above mentioned documents along with letter signed by both parents giving valid reasons for delay in registration of birth of newborn at the Consulate General of India, Dubai for prior approval. 

NOTE- If either of parent holds a foreign nationality, a letter from the concerned Embassy or Consulate of that country stating that the child has not acquired their nationality of his/her parents would be required.
